    If that fixed the issue then it seems like HD I/O issue on your server. Try this command on u Linux shell and dd if=/dev/zero of=test bs=64k count=16k conv=fdatasyncAnd post the output here. It might give an idea if something is wrong with hard drives.

    nexuz or nexus is cheat but pk3 in itself wouldn't cause violation. pk3 name or pk3 itself has no relation with violations.
